Mosaic’s Confidential Reporting Program (CRP) and Non-Retaliation Policy allow employees, people served, families, contractors, and others to report any issue that may be an ethical violation or a violation of the law without fear of retaliation. Reports can be emailed to or by calling the Compliance Hotline at 1-800-443-4899.

What is the Confidential Reporting Program?

Mosaic’s Confidential Reporting Program (CRP) may be used for reporting known or suspected violations and/or irregularities of the Compliance Program, Code of Conduct, laws, rules, regulations, and contractual agreements.

Examples of known or suspected violations to be reported to the Confidential Reporting Program:
  • Knowingly submitting false or improper documentation for services provided or expenses.
  • Knowingly billing for services not provided.
  • Improper handling of finances or property belonging to the people Mosaic supports or Mosaic.
  • Improper disclosure of protected health information (PHI), including loss, theft or unauthorized disclosure of PHI.
  • Electronic security incidents such as misuse or theft of computer equipment, unsecured electronic protected health information or unsecured Mosaic proprietary business information.
  • Any violation of compliance-related organization-wide policies.
  • Violations of the Code of Conduct.
The Confidential Reporting Program may also be used to contact the Compliance Department with any questions regarding potential violations.

To report a known or suspected violation to the Confidential Reporting Program:
  • Call the Compliance Hotline at 800.443.4899 (you can remain anonymous; a third-party vendor answers the calls)
  • Send an e-mail to:
I am afraid that I will be retaliated against if I report an issue, what should I do?
Retaliation of any kind is considered a serious violation of the Mosaic Code of Conduct, and Mosaic does not tolerate any form of retaliation.

Mosaic will not retaliate against an employee or individual because he or she reports any concerns or violations in good faith. Any such action is a violation of Mosaic’s Compliance Program and should be reported immediately by calling the Compliance Hotline 800-443-4899 or emailing Mosaic’s Compliance Department at Individuals found in violation of the Non-Retaliation policy and procedure are subject to corrective action, up to and including termination of employment.
