Licensure

REQUIREMENT TO NOTIFY NATIONAL OFFICE
 
The following activities of State and Federal regulatory agencies or funding sources must be reported to the Compliance Department
  • Routine and out-of-cycle licensing surveys and quality reviews, including revisits in response to a plan of correction;
  • Complaint-driven and investigations resulting from a reported incident;
  • Department of Labor/Wage and Hour reviews;
  • Billing audits; and
  • Receipt of written or verbal notification of a threat to revoke a license, not renew a license/certification/contract for services or a status of no referrals/admissions made due to quality concerns.
The following notifications or a survey, audit, or investigation are to be made to the Compliance Department via licensing@mosaicinfo.org.

Surveyor/Auditor/Investigator on Site - Immediate notification is made to the Compliance Department that an external surveyor/auditor/investigator has arrived at the agency must include
  • Date and time the survey, audit, or investigation began
  • Name and title of person conducting the survey, audit or investigation
  • State or Federal entity the person is affiliated with; and 
  • Purpose of the external survey, audit, or investigation
If there is a change in status or scope of the survey, audit, or investigation based on new evidence discovered at the time of the survey/audit/investigation, the Compliance Department must be notified immediately.

Surveyor/Auditor/Investigator Exits Agency - Email sent to Compliance Department must include:
  • Date and time of the exit;
  • Name and Title of person conducting the exit; and
  • Summary of findings shared during the exit that includes any follow-up activities discussed.
Statement of Deficiency (SOD) Received - Email a copy of the report and cover letter to the Compliance Department immediately when received.

Plan of Correction (POC) Required - Email a copy to the Compliance Department immediately once completed and submitted.

Notification that the POC has been Approved or Denied - Email copy of letter of approval/denial to the Compliance Department immediately upon receipt.

Survey/Audit/Investigation Follow-Up - Follow-up activities related to the statue of the POC will be determined by the agency Executive Director or designee and the Licensure and Accreditation Architect. Activities may include, but are not limited to:
  • Schedule for routine calls on status of implementation of the POC;
  • Submission of documentation to verify completion of the POC;
  • Site visit to verify practice of the adjustments made to systems and processes related to the POC; or
  • Arrange for access to Mosaic National Supports that would be beneficial to ensuring complete implementations of the POC.
